What does Cosmelan Treat?
Cosmelan is used to treat hyperpigmentation, a very common skin disorder. It presents itself in brown patches, often found on the hands and face, leaving the skin blotchy and uneven. It is caused by a genetic predisposition, hormonal influences (such as pregnancy, menopause and oral contraceptives) and exposure to the sun.

Hyperpigmentation is a very common skin disorder. It presents itself in brown patches, commonly found on the hands and face. In some cases it is referred to as: Melasma.
What causes Hyperpigmentation?
Skin tone is defined by the presence of melanin. Darker skins have higher amounts of melanin, and lighter skins, have lower amounts of melanin. Often, Melasma occurs when there is an increased melanin production. This creates a darker skin tone in some areas, leaving the skin blotchy and uneven. Causes of skin pigmentation can range from genetic predisposition – aggravated by high exposure to strong sunlight, to hormonal changes – such a pregnancy, menopause or taking oral contraceptives.
How does Cosmelan help?
cosmelan® acts by blocking the melanin-producing enzyme (Tyrosinase), therefore reducing the production of Melasma. A significant reduction in brown patches may be detected only one week after starting treatment. The appearance of new, luminous and rejuvenated skin is present, and skin continues to improve with the use of the maintenance products.

What is the cosmelan® procedure?
cosmelan® is a professional treatment and must be administered by a qualified and experienced practitioner. Both cosmelan® and dermamelan® contain the same active ingredients to treat hyperpigmentation, while dermamelan® is slightly stronger.
Phase 1:
Preparing your skin pre-procedure. The better your skin is prepared, the better the result and the lower the risk of inflammation or discomfort. The mesoestetic Brightening Booster is recommended to be used for 6 weeks prior to the the treatment, and mesopeel Melanostop 48 hours before the ask application will provide an added benefit.
Phase 2:
The in-clinic treatment which takes about an hour, and involves the application of the cosmelan mask. Once the mask is applied, you will be taken through the protocol carefully and you will be able to go home. You will be instructed to remove the mask after a specified period of time.
Phase 3:
The maintenance period over the next 12 months, which is critical to the success of the cosmelan treatment. You will be prescribed home care products and given careful instructions to follow to best maintain your result. Home care products during the first month include: mesoestetic’s Mesoprotech Melan 130+ Pigment Control, Melan Recovery and Cosmelan maintenance cream. During the next 11 months, the following products can be added: mesoestetic’s Fast Skin Repair, Stem Cell Serum, Aox Ferulic, Regenerance Active, Proteoglycans and Anti-stress Face Mask.

In many cases a reduction in brown patches may be detected as soon as one week after starting treatment. Once peeling is complete, the skin will appear more luminous and rejuvenated, and should continue to improve with the use of home care products.

Cosmelan Depigmentation treatment action mechanism is based on the inhibition of tyrosinase, a basic enzyme in the melanin formation process. Several of the substances present in the Cosmelan® formula act by blocking this enzyme, or even by inverting the metabolic process of the transformation chain. Melanogenesis comprises the following stages:
TYROSINE proceeds to DOPA by means of TYROSINASE.
DOPA proceeds to DOPAQUINONE by means of TYROSINASE.
DOPAQUINONE proceeds to PHAEOMELANINE and EUMELANIN by means of oxidation and cyclization

ONE WEEK after starting the Cosmelan® Depigmentation treatment, a noticeable significant improvement of the skin is observed! The reduction of the treated blemish is detected and the appearance of new luminous and rejuvenated skin is present.

Important: it must be known if the patient has undergone any previous treatments, especially with glycolic acid. If confirmed, any treatment, especially Glycolic Acid, must be eliminated at least, one week prior to using Cosmelan Depigmentation treatment.
The Cosmelan® Depigmentation treatment initiated at the medical office must be continued at home, on a daily basis, according to the guidelines indicated.
The treatment must not be interrupted under any circumstances, during the whole time prescribed.
It is completely forbidden to combine the Cosmelan® Depigmentation treatment with other depigmentation treatments, as the appearance of undesired effects cannot be guaranteed.
Treatment control visits are recommended. 8 days after beginning of treatment, 15 days after, and a follow up visit every month thereafter.
First and second day
During the first two days of the treatment, the skin may turn red. If the skin is very sensitive, the patient may experience an itching feeling. Slight burning and tightening of the skin is also absolutely normal, especially during the first 24 to 48 hours after starting the treatment. To relieve these problems, it is advisable to apply the Mesoestetic Hydra-vital factor K whenever necessary.
Reddening of the Skin. The reddening that occurs on the face after applying the Mesoestetic Cosmelan mask treatment is similar to the effects of day-long exposure to the sun in the snow without protection. (Intense reddening in photo type I and II and practically non existent in photo type IV and V).
Flaking may occur on all skin types (and more intensely in photo types I and II) between the second and third day. In this case it is advisable to apply the Mesoestetic® Hydra-vital factor K cream, repeating this every two hours on the day when the flaking is most intense repeating this every 2 hours until it improves. Do not pull on the flakes. Do not rub or scratch the skin. If necessary flakes can be cut off. Do not use exfoliants, scrubs or peels. Avoid steam baths, saunas and swimming in a hot spring for 2 weeks following treatment. It is advisable to wait until the initial flaking period is over before starting to apply make-up again.
Warning
If a severe skin reaction results from treatment do not proceed with maintenance treatment. Similarly, if scab formation is seen or if the skin is accidentally injured during the treatment period it is advisable to interrupt application of maintenance treatment. Use only Hydra-vital factor K and Sunscreen until the skin has settled and then again proceed with the maintenance treatment schedule. HOME TREATMENT – Cosmelan Maintenance Cream
The second phase of the Cosmelan consists of a home maintenance cream Cosmelan with the intention of securing the depigmentation by association of agents such as Kojic Acid, Phytic Acid, and Vitamin C. We have added a physical agent to block the sun and guarantee that the UV rays do not keep on aggravating unwanted cutaneous hyper pigmentation.
Mesoestetic Vitamin C ampoules: use twice weekly before applying the Cosmelan 2 cream.
Hydra Vital K: The recommended moisturizer.
Regenerance Active: recommended moisturizer for sensitive skin.
Mesoestetic Complete sunblock: Highly recommended for continuous use.
Melanogel Touch: this can be applied on heavy spots. Recommended for added benefit.
Melanogel Touch anti-spot cream: this depigmentation cream works very well to maintain results during stabilization period.
The treatment results are largely dependent on your cooperation and adherence to the post treatment skin care! The first 4 months constitute the treatment period. The period following is called the stabilisation period, and this is ideally extended up to one year, or longer. Every effort should be made to continue treatment throughout the first summer. Even if the depigmentation treatment was a great success, you need to understand that long term success depends on how well the protocol is followed, during both the treatment and stabilization phases. Avoid sun exposure for 2 weeks prior to treatment.
No peels can be done 2 weeks prior to treatment, No Laser treatment can be done on area to be treated 2 days prior to peel, Avoid Botox and Filler in area to be treated 2 weeks prior to peel.
